Personalized care can best be given in a good environment. In the best interest and safety of all of our patients, as of January 1, 2011, the following policies will be in effect:
• All appointments not cancelled within 24 business hours (1 business day) prior to the scheduled appointment or missed appointments, will be assessed a $35 late fee for each occurrence.
• Patients will ONLY be allowed one (1) guest to accompany them to an appointment.
• Children between the age of 3 months and 10 years old, will NOT be allowed in the office, unless it is the patient.
We apologize for any inconvenience this may cause. Guests that are unable to attend the appointment are welcome to wait in Jitters Cafe on the first floor of our building. We will be happy to re-schedule your appointment so that proper child care can be obtained.
• The $35 processing fee to have Disability Forms filled out will have to be collected at the time a patient requests to have the forms filled out or processed online. We will NOT be able to process any forms until the processing fee has been paid.
